Instead, it is a "skin disease" with clear pathophysiological mechanisms, and a thorough understanding of these mechanisms is the "first key" to effective and sustainable acne treatment.


Today, I will take you deep into the microscopic world beneath the skin to understand the "4 main mechanisms" that are the root causes of acne problems, as well as reveal the "contributing factors" that often fuel the spread and severity of acne.

Imagine our pores as small "volcano vents," with sebaceous glands at their base that naturally produce oil (Sebum) to moisturize and protect the skin. However, when abnormalities occur in these processes, the "volcano vent" is ready to erupt into acne at any time.

Origin from Sebaceous Glands 

Beneath our skin are "sebaceous glands," which are factories that produce natural oil, or "Sebum." This oil has an important function: to coat the skin, making it soft, moisturized, and creating a protective barrier. But for people prone to acne, especially those with oily skin, these oil factories work extra hard, producing more Sebum than necessary, making the face appear shiny, greasy, and leading to a condition called "Seborrhea."


Why is so much produced? 

Excessive oil production is often stimulated by androgen hormones, which are male sex hormones present in both men and women. This is why we frequently see acne during adolescence when hormones are surging, or in some adults with hormone imbalance problems.


Impact 

When there's an abundance of Sebum, the consequences include enlarged pores to drain the oil. Most importantly, this excess Sebum becomes an "excellent food source" for bacteria and "helps" dead skin cells to clump together more tightly, leading to the next mechanism...

Old skin cells that don't go anywhere 

Normally, our skin continuously sheds old, dead skin cells to allow new ones to replace them. This process takes approximately 28 days. However, for people with acne problems, the process of skin cell shedding in the pore walls becomes abnormal. Old skin cells that should have shed instead clump together tightly and accumulate inside the pores.


When skin cells combine with oil 

These dead skin cells combine with the excessively produced Sebum and various impurities, forming an "obstruction" called a "Comedone" or "clogged pore." There are two main types of comedones we are familiar with:

  • Blackheads (Open Comedones / Blackheads) 

    Result from a clog at the end of the pore that opens to the outside. When the comedone head comes into contact with air, oxidation of fats and skin cells occurs, making the comedone tip appear black; it's not because of dirt, as is often misunderstood.

  • Whiteheads (Closed Comedones / Whiteheads) 

    Result from a clog at the end of the pore that is completely closed beneath the skin. They appear as small white or skin-colored bumps. If not properly managed, whiteheads are more likely to become inflamed acne than blackheads.


Bacterial Pathway 

This clogging not only makes the skin look uneven but also creates an "ideal environment" for bacterial growth, which leads us to the third mechanism.

Commensal Bacteria 

Everyone's skin naturally hosts a type of bacteria called Cutibacterium acnes (formerly Propionibacterium acnes or P.acnes). This bacterium usually thrives in anaerobic (oxygen-free) conditions and feeds on oil (Sebum).


When the environment is favorable 

When pores become clogged with a large number of skin cells and Sebum, the environment inside the pores becomes anaerobic, which P.acnes favors, and it has abundant food. This causes this bacterium to "multiply rapidly," turning from a friend into an enemy.


Releasing inflammatory substances 

When P.acnes proliferates excessively, it produces various substances and enzymes. These substances then trigger an "inflammatory reaction" within the pores. This is the beginning of "inflammatory acne."

Combat Mechanism 

When the body detects an obstruction and an abnormally high proliferation of P.acnes bacteria, it sends "immune cells" to deal with them, to destroy the bacteria and foreign matter that are clogging the pore.


Symptoms 

This immune system combat process is what causes "inflammation," which we can observe as swelling, redness, and pain around the acne lesion.


From Clogged Pores to Various Types of 'Inflamed Acne': 

  • Papules 

    Small inflamed acne lesions with no pus head, slightly painful.

  • Pustules 

    Red bumps with a yellowish-white pus head in the center.

  • Nodules (Large Inflamed Acne / Cystic Acne) 

    Inflamed acne lesions larger than normal, appearing as hard lumps under the skin, causing significant pain due to deep inflammation reaching the dermis.

  • Cysts 

    Large inflamed acne lesions that appear as fluid-filled or pus-filled sacs under the skin, often interconnected with multiple lesions, and causing severe pain.


Subsequent Damage 

If inflammation is severe and prolonged, it will result in "damage to collagen" and underlying skin tissue, which is the main cause of subsequent acne scars and acne marks.

In addition to the 4 main mechanisms mentioned above, from my perspective as Dr. Nono at Entrio Clinic Ratchaprasong Aesthetic Clinic, I also see several "contributing factors" that act as "fuel," causing each person's acne problems to differ and making acne treatment more complex:


1. Hormonal Fluctuations:

  • Teenage Trouble 

    Increased androgen hormones during adolescence stimulate sebaceous glands to work harder, making acne more likely in both males and females.

  • Adult Acne 

    In adult women, acne may flare up during menstruation, pregnancy, or in conditions with hormonal imbalances such as Polycystic Ovary Syndrome (PCOS), which often manifests as acne around the chin, mouth, and jawline.


2. Genetics: 'Family Acne'

If your parents or siblings have a history of severe acne, you are also more likely to develop acne easily and severely. Genetics plays a role in determining the size and function of sebaceous glands, as well as the skin's response to various factors.


3. Stress: 'Anxiety Acne'

When we are stressed, the body releases cortisol, a stress hormone. This hormone can stimulate sebaceous glands to produce more oil and also affects the immune system, worsening inflammation and slowing down the skin's repair process.

Although widely debated, based on observing many patients, I have found that certain foods may worsen acne in some individuals, especially:

  • Dairy Products 

    Especially cow's milk, may contain substances that stimulate the production of hormones related to acne.

  • High Glycemic Index Foods 

    Such as white bread, white rice, and sugary drinks, may stimulate insulin and IGF-1 secretion, which in turn stimulate Sebum production and inflammation.


Adjusting dietary habits is therefore a part of the holistic approach to acne problems that Dr. Nono emphasizes.


5. Comedogenic Products (Cosmetics and Skincare Products): 'Helper Turned Villain'

Using unsuitable products or those with comedogenic ingredients (that cause pore blockage) is a significant cause of new blackheads and inflammatory acne.

I often compare choosing skincare for acne-prone skin to "cooking." You need to select good ingredients, prepare them in the right proportions, and follow the steps meticulously. Only then will the result be delicious and good for your skin's health. If you choose incorrectly or use them improperly, instead of improving, your skin can easily worsen.


Many people with acne problems often mistakenly believe that they need to use strong products to dry out acne quickly, or that the drier and tighter their face feels, the better. This is a dangerous and absolutely incorrect belief! Doing so will not only fail to clear your acne but may also damage your skin barrier, making your skin more easily irritated and sensitive, and stimulating your sebaceous glands to produce excessive oil to compensate for the dryness, leading to a vicious cycle where acne never truly clears.

  • Non-comedogenic (Does not cause clogging) 

    This is the most important key for acne-prone skin! Every product you use, whether it's a cleanser, moisturizer, sunscreen, or even makeup, should always have "Non-comedogenic" or "Non-acnegenic" indicated on the label. This means the product has been tested and found not to cause pore clogging, which is the main cause of blackheads and inflammatory acne.


  • Oil-free (Free from oil) 

    For people with oily and acne-prone skin, choosing oil-free products will help reduce the burden on the skin, reduce stickiness, and reduce the risk of clogging better than products with heavy oil ingredients.


  • Fragrance-free / Alcohol-free 

    Fragrances and alcohol used in many products are excellent "triggers" that can easily irritate sensitive skin and worsen inflammatory acne. Especially for sensitive skin, avoiding these ingredients will greatly protect your skin.


  • Gentle 

    Always remember that "stronger is better" is not the principle for acne-prone skin. Once you understand the principles, let's organize your skincare routine. I will divide it into 4 main steps that must be done consistently, both morning and evening:


1. Cleansing: 'Wash... but Don't Harm the Skin'

  • Goal: To remove dirt, excess oil, makeup, and dead skin cells without damaging the skin barrier or making the skin overly dry and tight.


  • Choose: 

    Gel or foam cleansers that have minimal lather, or are foam-free (non-foaming cleanser), are even better, as excessive foam often contains surfactants that can be harsh on the skin.


    Oil-free and Non-comedogenic formulas: Check the label carefully.


  • Recommended Ingredients: 

    • Salicylic Acid (BHA): In appropriate concentrations (0.5-2%), helps to exfoliate dead skin cells and dissolve oil clogs in pores, reducing the formation of blackheads and also slightly reducing inflammation.

    • Zinc PCA: Helps control oil and reduce inflammation.

    • Gentle Surfactants: Gentle cleansing agents such as Coco-Betaine, Decyl Glucoside.


  • How to Use: 

    • Wash your face only twice a day (morning and evening). Washing your face too often will stimulate the sebaceous glands to work harder.

    • Use room temperature water. Never use hot or very warm water, as it will make your skin drier and more irritated.

    • Gently massage the cleanser all over your face, especially in areas with acne and large pores. Use your fingertips to gently rub in circular motions; do not scrub vigorously.

    • Rinse thoroughly with clean water, then gently pat your face dry with a clean, soft towel (use a separate towel specifically for your face and wash it regularly).


  • Things to Avoid: 

    Cleansers that make your skin feel "squeaky clean" after washing (this means you have stripped away natural oils and damaged your skin barrier), and vigorously scrubbing your face with a brush or loofah.

    • Benzoyl Peroxide (BP): An effective medication for killing P.acnes bacteria and helping to reduce clogging. Available in concentrations from 2.5% to 10%. It is recommended to start with the lowest concentration (2.5%) to reduce irritation. Often used as a spot treatment for inflamed acne 10-15 minutes before washing your face, or as directed by a doctor.

    • Retinoids (Vitamin A derivatives): Such as Adapalene, Tretinoin (prescription medication) or Retinol (for general use) are "gold standard" in acne treatment! They work to normalize skin cell turnover, reduce pore clogging, reduce inflammation, and also help reduce acne marks. Should be used at night, starting with less frequent use (e.g., every other day) to allow the skin to adapt, as irritation may occur initially.

    • Azelaic Acid: Helps reduce inflammation, kills bacteria, reduces the formation of clogged pores, and notably helps reduce red and dark acne marks. Available in cream or gel form.

    • Niacinamide (Vitamin B3): A gentle but powerful ingredient that helps reduce inflammation, reduce redness, control oil, tighten pores, and strengthen the skin barrier. Can be used both morning and evening.


  • How to Use: 

    Apply treatment products after cleansing, either all over the face or as a spot treatment, depending on the product type and recommendations. If using multiple products, start with the lightest texture first, then follow with heavier ones.


3. Moisturizing: 'Adding Water... Reducing Oil... Building a Skin Barrier'

    Gel or lightweight lotion moisturizers that absorb quickly and are non-greasy. Oil-free and Non-comedogenic formulas are extremely important!


  • Recommended Ingredients: 

    • Hyaluronic Acid: An excellent humectant that draws water into the skin without causing oiliness.

    • Glycerin: Another gentle and effective humectant.

    • Ceramides, Cholesterol, Fatty Acids: Important components of the skin barrier, helping to restore and strengthen the skin, making it more resilient to external factors.

    • Panthenol (Vitamin B5): Helps soothe the skin, reduce redness and itching, and promote skin repair.


  • How to Use: 

    Apply moisturizer immediately after applying treatment products (and serum, if applicable) both morning and evening. Apply an appropriate amount, not too much, to avoid a heavy feeling on the face.



    Tip: Apply when the skin is still slightly damp after washing, as this helps the moisturizer absorb better and lock in moisture.

    Sunscreen with SPF 30 or higher and Broad Spectrum properties to protect against both UVA (causes wrinkles and dullness) and UVB (causes sunburn) rays. Oil-free and Non-comedogenic formulas are most important for acne-prone skin!


    Gel, lotion, or fluid textures that absorb quickly, leaving no greasiness or white cast. Physical sunscreens (Mineral Sunscreens) containing Zinc Oxide and/or Titanium Dioxide may be a good option for sensitive skin as they are less likely to cause clogging than chemical sunscreens.


  • How to Use: 

    Apply daily in sufficient quantity (about two finger-lengths for face and neck) in the morning, as the last step of your skincare routine. Reapply throughout the day if necessary, especially when outdoors for prolonged periods, sweating heavily, or after swimming.
